Activity ​Participation Agreement

Some activities require a guaranteed number of attendees with pre-payment; in which case, refunds will not be available.  On rare occasions, limited space may be available.  Please sign up early and call immediately if the student is unable to attend so the opportunity can be offered to another student on the waiting list.  Refunds may not be available if the student is late arriving for activities that require participants to go in as a group.

A release of liability form is required for all participants both students and guests for participation in all PSP activities. Your student will have completed this form upon enrollment. All guests will need to submit a completed form to the PSP office prior to participation. Contact our offices for more information

If a student causes bodily harm to another person or causes property damage, that student’s parents/legal guardians are responsible for any compensation required.  ​
