Elementary Evaluation System
There are 3 different forms for Elementary Evaluation; graded, written and unit study. You can use whichever of the three options you feel most comfortable with. If you have any questions do not hesitate to contact the PSP Office for more information.
Letter GradeIf you would like to provide the traditional letter-grading evaluation of your student, please submit this completed form at the end of each semester

Written EvaluationIf you would like to evaluate your student's progress with a written description, please submit this completed form at the end of each semester.

Unit ProgressIf you would like to show the progress made using the unit study method please submit this completed form at the end of each semester

Elementary Course of StudyYour Course of Study outlines your student's curriculum and course plan for the year. Your student's course of study must be submitted to the PSP office for each student at the start of each semester.
A general course of study for elementary would include Mathematics, Social Studies, Science, and Language Arts. CCPSP requires Bible as an additional subject. |
Attendance RecordA Record of Attendance is required at the close of each semester for each of your students. Please download and complete the below form for your Elementary School Student and submit to the PSP staff.
